Patient was to have ground food and was given a regular diet. Patient fell several times and when pain was reported, patient was told it was all in the mind - nothing was wrong. Patient had the same sheets on the bed for 14 days in a row. A bottle of open urine from roommate sat on the floor of the room the entire day. Nursing staff asked patient's spouse where to put a medication patch on the patient. Office staff constantly badgered family for payment after family explained that payment for services supposedly rendered was on the way. Therapy staff was excellent.
