My mother was transferred to Forest Hills for rehabilitation from a fall. The staff physician's care was bad. The physician, for no reason, stopped mom's pain medication and this immediately put her in distress. Upon finding this out a day later, I questioned the staff. They knew this had happened along with the level of distress this put my mother in but did not question the physician or inform the family. Do NOT put a loved one in this facility. The administration was completely apathetic to what happened. Be very careful with this facility.

Details:

Awful place. They go against the PCM's orders. Change his meds even though they assured us they would get prior consent from the family before hand. Keep moving my grandfather from room to room. Getting him out ASAP!!

Details:

This facility has left my mom unattended in the bathroom where she fell off the toilet. They are under staffed and some of the staff have little or no compassion ofr the elderly. I am looking for a different facility to take my mother to. If I could care for her at home I would do so, but I am not in a position to do so.

I placed my mom here after a broken hip and she received excellent care and rehab. The staff was nice and the facility clean and no smell. She loved her therapists and kept in contact after her discharge by visiting. She is now home and living on her own. I would highly recommend this facility. They have made changes in staff and management this past year and I think the poor reviews in the past don't reflect the care given now.

My father was in this rehab center for only two weeks and contracted c-diff and pneumonia there. The on-staff Doctor said she would give him medicine to combat the diarrhea but never did. She also stated she would give him something for his appetite and didn't until the day before he went to the hospital. One night, I arrived for my evening visit and my father was unable to put his fork into his mouth or keep his head straight. It appeared as though he had a stroke. The center was refusing to transport my father to the hospital until I demanded they call a transport. They assured me that we would be returing later that night and there was nothing wrong with my father. Upon arriving at the hospital they immediately found that Dad had pneumonia and c-diff. He died in the hospital after fighting the infections for for six days.

My father was a resident. He was there for physical therapy following a stroke. He had little bowel control. He developed a severe case of what can best be described as diaper rash that became open wounds. The wounds became infected because he sat in his own filth most of the day until a family member came to visit and had him cleaned up. After just a few weeks at the place the infection had spread. He was found unconscious and his organs had begun to shut down. He was admitted to a hospital, place in intensive care and died a few days later.

Details:

We placed my mother in the facility for rehabilitation and a year later she is still there. We chose this facility because it had Alzheimer's care and rehab. They would not rehab her because she had Alzheimer's. Hence, if they didn't do a good job rehabbing her, then they had a patient for life. What a crock. They are under-staffed most of the time and there is a good deal of turnover. The care is adequate at best. They even took my mother off of medicare earlier than the 100 days provided because they couldn't provide rehabilitation. I would never do this again. They are overly focused on things like family members getting ice, but don't worry about the grime on the floors or tables not being scrubbed.
