Details:

Due to the utter lack of basic care given my Father he was sent to ICU. I have contacted the corporation many times to have my Fathers medication given in atimly maner and was told the problems would be fixed. Never were the issuses fixed. Not only did my Father feel unsafe in there care they did real harm to his overall health. I would not trust them to care for a rabbid dog. If you love someone who is sick and need of care do not let them suffer at this smelly den of horrors.It is the three layers of HELL! I am contacting the state to help save others from the pain they put on our family.

Details:

Care is being provided for a 90 year old male. There are some good nirses and good LVN's at the facility. Overall, the right hand does not know what the left hand is doing. The administration has no clue as to performance of staff. Cleanliness of the individual is poor. Instructions from family and care providers is rarely folllowed. They do not help feed the individual even though there are days where it is necessary. Consequently, there is significant weight loss. Health issues are not reported to family or power of attorney. A doctor's appointment, scheduled by the facility is unknown to the people who take care of the patient, therefore, the appointment is missed. I would not allow my dog or cat to reside here.

Details:

I could not believe the treatment my father received at Avamere at Three fountains. They gave him medication that he was alergic to, the male personal grabed him on his injured shoulder. When I flew down to see him there they had him transported to Providence with a possible stroke, which really upset the family. All reports came back negative. I would not recommend this facility to anyone. My fathers cloths ended up with other people's names on them with other people wearing them. What confusion takes place here!!!!

The nursing staff doesn't pay close attention to the residents. They don't offer drinks between meals to resident who could not get her own. Would not toilet patient routinely when pt could not make needs known. When told the nurses pt was continent is all they would need to do is offer toileting schedule. They just put a diaper on resident. Pt. fell family requested x-ray and did not do it. Stated that therapy said it was not necessary. Pt had hematoma on back of head. appeared to be changing details of incident. Resident has not had good pain control while there either. Patient was put on pain patch which came off in the shower. Staff refused to apply a new one they stated it was against fda regulations . When checked myself it should have been noted by 2 witnesses it was wasted and another put on. Also I requested since they would not put the patch back on, that they make sure to medicate the patient with prn med as ordered to keep the pain under control..I came back the next day in the morning the patient was in a lot of pain. When speaking with the morning nurse she said the patient should have been given the patch when it fell off. When asked if she recieved any pain med the night before it was said she didn't have any pain med at all. It took most of the day to get the pain under control. Also it has been requested many times to give her pills whole and yet the nurses continue to crush them knowing she will spit them out. She has some critical drugs she needs. They don't seem to care. Also when giving the meds one nurse always just gives them quickly, what I mean by this is she does not explain first what she is [planning on doing. She just shoves the spoon of crushed meds in her mouth. Its startling. Also she trys to give it to her while she is trying to eat, which spoils her taste for what she is eating. I feel she has gotten poor care. When we first got there I thought the care was fair but after watching the patient and others in the facility I am not impressed. There were also many times I asked for pain meds to be given and they never bothered to come in with the med or tell us if it couldn't be given. I think the patient should at least have their pain kept under control.

My grandfather went into this home after having a stroke. Between my Grandmother, Mom, Aunt, Sister, and me we were all there from the time he woke up to the time he went to sleep at night. what we observed while in the rehabilitation part is that they were extremely understaffed. You would call the nurse call button and it would take then any where from 30 minutes( at the earliest) and up to an hour sometimes more.... to come help you go to the bathroom or with anything else you might need. They did not care about the patients and they said they did self feeding but we would purposly leave for 20 minutes when we knew food would come and they said they would feed him... when we came back his food would be sitting there cold. It was so sad and so taxing on us to watch him be in such a horrible place. We literally didn't feel safe leaving him there. They don't care about you and are just way understaffed for 6,000 dollers a month they did nothing for him and he sat in a dark room, just so sad, same with his room mate. The poor guy would wet himself all the time because they just didn't have enough staff, or the girls were out in the hall talking about stupid stuff and not doing there job. If you love your loved ones I beg you don't leave them here inless you check it out...and more than the introduction.....(THEY HAVE YOU SIGN SOMETHING SAYING IF THERE IS A PROBLEM OR THEY DO SOMETHING WRONG YOU WONT TAKE THEM TO COURT)---Super fishy.
