Then, I wish reserve some dialysis for my trip in Louisiana in your clinic if it's possible for this followings days please : monday 28 th february 2011 wenesday 2 march friday 4th march monday 7th march wenesday 9th march friday 11th march Can you tell me please the cost for this dialsis in your clinic ? I've need this information for the catch of load by the health insurance in France. Could you send an estimate to me please ? My cousin who lived in Louisiana have taken this informations near you : Kidney Care informs me that you will have to fax them, 2 months prior to travel, a list of your medications, a demographics sheet (?), your last 4 flow sheets (?) and within 30 days of travel, any recent blood lab work.

Thus I will warn my doctor nephrologist to send all the medical informations to you relating to me.

Perhaps do you have a form to supplement which you could send to me ?
