Details:

My mother has Alzheimers and uses a walker. Every time I visit, she is wearing a dirty diaper. She is the last person left in the dining area where she is found sitting at the table looking at her cold food. When I go at mealtime and feed her, she eats every bite. Recently asked for buttermilk at bedtime, and was told that they were "all out", having used what they had to make the cornbread for supper. The nurse aides are sweet and helpful, but they are too few in number for the number and needs of clients. Her roommate rings for help for my mother, and tells me that they "come when they can", often being very slow to respond. Hygiene articles left at her bedside are put away, and do not appear to have been used from one visit to the next.I have asked to be notified if my Mom goes to the hospital or doctor, so that I can accompany her, when I arrive, and went to Mom's room, finding her gone I was told by a nurse at the desk that it was not their responsibility to notify me.
