Details:

Please don't ever put a loved one here. Death certificate has wrong cause of death listed. Lackadaisical attitude with helping to have it corrected by attending physician. I had autopsy done. Pulmonary embolism caused death. Wanted to know what happened on that last day without success. Never return phone calls and have the most unprofessional people working there.
