Details:

My mom recently fell and was unable to go home. We choose Three Oaks for its excellent Physical therapy. No one wants to put their mom in a nursing home but we were happy with her care. Staff was wonderful. She has since moved to Assisted Living and still holds to go home.

Details:

It was a struggle from day one. Resident wore a brace for a compressed vertebrae. Resident told staff how the Dr wanted it put on, but no one listened. Meals were not on a schedule. Medications were to be given with food. Many times that did not happen, resulting in the resident becoming ill. Therapy was given at all hours, sometimes very early in the morning before the resident was up, or had eaten. Resident was sent to the clinic for an appointment they did not have. The resident told the staff she did not have an appointment, but no one listened or even checked to see if that was true or not. The family had signed a paper indicating that the family would transport the resident to all appointments. When a discussion of this error was held, the staff was unable to locate that form. The last thing we dealt with was medication given every day. This medication is only good for 28 days, but it was already 31 days when the resident was discharged. The Dr had provided a prescription for a refill, but the staff failed to get it filled.
