Details

Here is list of incidences our family member has suffered at 5 Oaks: • Fall risk - no bed rails provided even though our family member was in for rehab of a replaced hip. • Family member was left sitting in dining hall alone and was suffering a stroke. We only found out about it when we entered to visit. No telling how long our family member would have sat there. • Family member left in the bed during the day for an indeterminate amount of time. Our family member has had dried excrement on their backside when we've visited. • Staff not capable of reapplying protective footwear to help our family member's heal pressure sores from healing. Our family has had to instruct them how to use and to make sure they have been applied each day. • Inability to work with dementia and Alzheimers residents. Custodial staff do not seem to understand how to communicate and assist our family member with dementia. Nursing staff doesn't seem to instruct staff how to do that either.
