This company is late or a no show atleast once a week. The aides are for the most part addiquit, the manager is rude and full of excuses for why the company cant have aides show up when they are suppost to and they never can find any aides to fill in on short notice, even tho we have talked to aides that were never asked to assist. The office staff is a Joke and will tell you what you want to here but then do something else. There is no comunication between the staff and the aides...

The Staff was kind of mean! She did not let me know the stuff that I needed to know when I needed to know it at that time. My caregiver was great, she does her job perfect I just with that she could have found another job an could have had me to. My son is wanting to find another home health for me to use.

my mother has had home health for awhile now with yall, her caregiver is GREAT, but the office staff is rude. Plus the lady that answers the phone, shes a young girl cant spell her name but I do know that it starts with a T. I hope that my mother goes to another home health..

RN's did not practice very good patient care. Leaving me to mak eit to the shower without assistance or supplies after a total hysterectomy. Not conscientious about patient needs after surgery.

my mother was in the emergency room and a nurse came in and told her to close her mouth when she coughed well she was coughing and vomiting the nurse came back in and told her I i told you to keep your mouth closed and she put a mask on her I would like to see her cough and vomit with her mouth closed that would be amazing!!!!!

It seems as if making money is managements concern over residents or employees. They have caring staff but constantly under staffs care takers. They only time the work with enough is if they know state board is coming in...then they over staff for that day only! It's rough on the employees and the residents. Employees are not allowed to report it to state board in fear of losing their jobs. And also to add...the managing employees that are listed falsify documents. They are by law to work at least a minimal of 8 hours a year as an STNA...taking physical care of residents in order to keep their license. That's it--just a minor 8 hours (1 full shift as a STNA ) per year and they don't. They just have someone sign off for them as if they did. I would give no stars if I could.

Horrible place for your family member. The nurses are condescending, the staff "yes" you until they turn blue but nothing gets done! High turn over rate because they can't seem to keep good caring nurses. They don't properly educate staff and family members. Bed linens do t get changed, you have to fight with them to bath your family member. Resident beg for a cup of coffee and get turned away. Activities get canceled often. All nurses and directors ignore call bells! A nurse was informed about a resident slurring her speech and the nurse ignored this, next morning the resident was sent out due to a stroke! One other nurse discusses her personal sexual life in the hall. They walk around with cell phones and flip flops and once the state arrives they all scatter and act appropriate! This place is the dumps!

Citehealth really doesn't know what they're talking about. No emergency services? Geez, I wonder what I was doing and getting paid for for the last six years. I wonder if the ER docs at Hendrick in Abilene were hallucinating all those patients transferred there for specialty care we did not have available? (small town, two doctors) I guess there are several people who are delusional in believing they are still alive and walking around. There is an emergency medical group providing ER coverage there as we read citehealth's drivel. How can you justify your existence, citehealth? Wrong reports about stuff you clearly never checked. I'd never heard of you 'til now. You sure as hell never talked to me in the ER. Obviously, you never talked to the group, either. Explain my rating? We were, and they are, just good folks trying to make things better in a hospital which receives no tax support, and covers a medically underserved area that has been a federally declared disaster area for (I believe this number to be correct) 13 years in a row. I'd really like to see citehealth make things work as good or better, under the same circumstances. I live hundreds of miles away from there, and I'm needing some serious surgery. I plan on going there to have it done. I trust them with my life. They saved my life once already. They can and would be happy to do it again.

Emergency services appear to be very overburdened. Wait times are often very long. Emergency beds are crowded, cramped and service is poor. The banter of staff and EMT's is often loud and inappropriate. Wait times within facility are long.

I would avoid the Blaine location if at all possible. Their Intake coordinator uses deceptive behaviour to manipulate both patients and parents. I believe he could overcome his performance issues but only when he recognizes he has a problem. He does not understand the collateral damage in his path due to his poor communication skills The saddest issue is he isn't honest with himself.

Anthony Louis Center in Burnsville did a great job helping us with our son. We didn't know what to do, and their wonderful staff helped us through a very difficult time, from the assessment through treatment and aftercare. The family groups were excellent, especially thanks to the counselor Rob. We would recommend ALC to any family.

This company provided a service to a resident in northwest houston. One of the aides stole from the patient, we reported the incident and the supervisor had another aide look through the house trying to prove us wrong. This was cleary a invasion of privacy and trust of the patient. This company was terminated by the patient, but the company kept sending out aides and were refused upon arrival. I suggest putting in cameras into the home for security of all while the aides are around.

My husband has been in this nursing home almost 2 weeks from a e-coli urinary infection he got after a prostate biopsy which showed he has cancer. He also came down with Pheumonia. On top of all that he is disabled by MS. He had diahrrea a week at this nursing home before I learned that he was being given an ex-lax type rx med. He should have been monitored and taken off those meds. He ate almost nothing because of an antibiotic to treat pheumonia. When I asked the nurse she said there was no indication he had either lost weight or wasn't eating. I pried further and found that tracking his intake showed he ate 80% of his meals; in my observation it was more like 8%. Their record keeping was completely inaccurate. He ended up 10 lbs less than his normal weight. I opted to do his laundry at home. Twice there was feces in his shorts when I got home. They hadn't bothered to rinse them out. A couple very rude nurses. However, EXCELLENT REHAB PHYSICAL THERAPY. Nevertheless, I'm taking my husband out of there asap. I wonder if there are any nursing homes that are really good. The reviews of this one on most websites show a 4 star rating. I don't believe the inspections of nursing homes can be trusted.

I sent my love one to this place a month ago,this place is disgusting. The staffs were very unfriendly to begin with, I was standing at the counter to request for some information,they just ignored me for totally 15 minutes.They have a lot of staffs passing me by and at least 3 nurses sitting over the counter,but they did not look at me and ask me what i want. I was standing in front of this nurse for more than 10 minutes and she was totally ignored me,all she did was yelling at the phone. My love one was in pain because he just came out from surgery. We were requesting for pain medication, but all they said is pharmacy has not send the medication to them. My love one was without pain medication for two days, that is terrible! Everything we asked, we never get a direct answer and get a cold shoulder. For the whole week, I only saw the doctor came to our room once for 10 minutes. The nurse were very rude,they can not even tell me what medication that they are given to my dad. they came in without greeting and told my dad to take the medication quick because they have other patients. I really dont like this place. The other day, I heard the next door patient was crying for help because she wet her bed, she was eventually cried for help and slept on a wet bed for totally three hour until her family member came in and demanded the nurse to do something. I dont know, i do not like this place, if you have family sick and need to be taken care of,I dont think this is the right place for them.

The following review is just my opinion, I am not in the medical field at all and these views are just my belief on what I think/experienced: The facility was clean on each visit. Our loved one was very rarely dry on each visit-most of the time he need to be changed. Some of the staff were great; however, there was not enough staff to take care of the residents. They listed themselves as able to take care of dementia/Alzheimer; however, we found that rarely were the residents given water in what I believe to be a proper manner. Sure water was delivered to the room but how is a patient who can barely move able to obtain water? A lot of the time the water would be placed across the room. If you are familiar with dementia/Alzheimer patients, these patients need to have water PLACED in their hands and they need encouragement to eat/drink SEVERAL times a day. Even if water was within reach, some of these patients would not register in their brains they need water-you need to place it in their hands (my personal belief)!! Our loved one was dehydrated and had bed sores. The excuse given for bed sores "well if they don't move they will get bed sores" Well, duhh!! Its the nursing home responsibility to reposition the patient. Again, not dementia/Alz orientated. We were told by two different people who frequent different nursing homes that this place was one of the best-if that is the case I would have hated to have to deal with a worse one. Really all they need is more staff and to properly water/reposition patients. The staff are just to run down with too many patients, IN MY OPINION.

I would not put a loved one in this place, unless your okay with them being left alone, not eating,having NO AC on in there room.The NR's not reading there charts to give them the right med's. My advice find somewhere else or take of them your self.Your loved ones might not make it out of this place.

Apex Behavioral in Woodhaven is the most poorly run office I have ever encountered, and I have finally had enough. I work 50+ hours a week, so my psychiatrist usually just writes my prescriptions for me and leaves them at the front desk. I have done this a couple times before, and never had an issue. While the staff at this office will treat you like garbage, at least my psych was understanding enough to know my work schedule allows me to do hardly much of anything. I had called the office yesterday afternoon to find out if he could write a prescription out for me. They made note of this and said he would be returning to the office today and they would call me back. They called and I discussed a good time for me to head to the office to pick it up. We agreed on 7:30pm, which was going to result in me having to leave early from work by 2 hours, and at my job, any hours that are missed MUST be made up before leaving the office on Friday. I cannot leave with less than 50 hours on my time card (unless I have left sick or something along those lines. This was not an excusable reason to not make up my time). I got to the office at about 7:10, stood at the front desk for 15 minutes with no one in sight, even walked to the back to see if I could find anyone. No one to be found. So I take a seat, and wait another 40 minutes before a therapist comes out and I ask them if they know where someone from the front desk is and they tell me they have already left for the day. I explained I was only there to pick up a prescription and they were aware I was coming at this time (I had even gotten there early), and I asked her if she, or someone else could help me. She said no and she does not know where they keep the prescriptions. Ok, thanks for nothing. So now not only do I have hours to make up at my job for absolutely nothing, but I also do not have my prescription and have an endlessly bad taste in my mouth about this office forever now. If we hadn't agreed on a specific time, or if I wouldn't have had to leave my job early, I could have looked past it. But this was not the case. I don't recommend this office to ANYONE. They DO NOT care about you nor your well being. Not to mention, one of the younger guys that works behind the desk is everything but a ray of sunshine and constantly sounds miserable. Seems like he needs to be one of the patients rather than an employee. FIND A BETTER OFFICE!

Been going there a long time. Anti Depressents and anti-anxiety pills are given out freely and the Dr. has no desire to ween me off. I complain that I want off, but I still am given more. As far as counseling, I was told to look up info on internet and two of the counselors just sat and yawned while I talked. No help at all !!! Go somewhere else if you really want someone who is concerned. I am looking now.

I was court ordered to enroll in this treatment center. After the initial orientation/evaluation, one month has passed and I've have not been informed on when or where my treatment begins. Several follow up calls to the agency have not provided any results. Some employee's of this agency do not return calls as promised. This make for a most uncomfortable situation as the potential for re-incarceration exists.

Trash in the hallways and in patient rooms. Teh room was a mess. The call button is of no use, no one responds. Patient hygene is not addressed. Staff has horrible attitude. Patient was in a week before therapy began.

I could not figure out how to give a zero star, or several would have been zero. I cannot say anything about the facility, we were never there. The quality of care was lacking in very obvious ways. Simple things like Blood pressure never being taken. The facility expected my Dementia father to be able to handle his own scheduling. They constantly used his inability to answer the phone as an excuse to not show up. Several times the nurse had a family emergency or car problems. I would fired this company, had the VA not contracted with them.

This place may have gotten a 5 star rating because of their rehabilitation techniques but it should get an overall score of 0.5 stars. The staff here is atrocious. My father was in here to rehab his knee. All night long you hear screams of the patients that go unanswered. The nurses are very lazy and clearly do not like their job. Nothing in this place works. In my fathers room, the call button was not functioning so they gave him a bell to ring if he needed them. Only problem is they also shut his door and no one could hear the bell. The next day maintenance tries to fix it and assures us that it is fixed but it breaks again in less than 2 minutes. The call button in the bathroom also did not function. The heater in the room only blows cold air so they brought in an electric heater that barely raised the temperature 5 degrees. It was freezing in that room. We were finally able to get him into another room. The nursing staff has no idea how to administer medication and were caught (by us) on several occasions giving my Dad the wrong medicine. This alone should send everyone running to another rehab center. The place is very run down. I would not say dirty but very unkempt. Thankfully, with the help of a very good social worker, we were able to get my Dad into Huntington Hills. If this is your only option for rehab, I would just go home and hope for the best because you will not get good care here.

rehab excersize was in contradiction to dr and hospital recommendation, I needed rehab form their rehab. the Dr discontinued my water pills which is the most important of the 18 pills a day I take and could be fatal considering I have congestive heart disease. The management is nasty. The food is always cold and if they spend $1 per meal I would be surprised. I could have received better care at my dog's kennel.

My sister was taken to Apex from the hospital on Friday 5/17/13 to get rehabilitation for a badly fractured ankle after 2 surgeries. Her first day there the Nurses Aid rammed her bad foot into the wall, blamed my sister for it saying that she tried to get up too soon & then rammed her foot into the wall a 2nd time. When I called to complain a supervisor Darry said he would check it out & got back to me. He did call me back but had been unable to speak to the aid but she would be spoken to. That night - even though she stated upon her arrival that she is "continent" an aid brought a 61 year old woman a diaper!! The following morning upon being helped into her bed she fell because the bed wasn't braked. She fell on the bed but her bad foot hit the floor!! I called & spoke to a supervisor Shannelle about both the diaper incident & also the fall & was told she would speak to my sister & call me back - she never spoke to my sister & never called me back!! That night they brought her a "pullup" to put on!!! It seems that patients are not safe & there is NO dignity of care!

Nurses do not listen to resident when they inform them that they gave them the wrong medications. Nurse told resident to shut up and just take the dam pills. Social work department needs to review resident rights! SW does not keep the resident or family informed about discharge plans

Quality of care at this facility is extremely poor. With the exception of some good nurses, staff and CNA's,direct patient care is not pro-active and many get by by doing minimal requirements. Residents seem frustrated and so are their family members. Shifts,particularly in the evening and on weekends are short staffed for a variety of reasons. This happens frequently and is more the rule than the exception. Conditions have not improved inspite of formal complaints.This nursing home from ownership to management on down is lacking an emotional investment in the care and happiness of their residents and it is visible and observable to those who spend time visiting in this facility.

The nurses were very kind, treated my Mom like a V.I.P. I especially loved the one on one attention and care given no matter what time of day, or day of the week. Everyone that came to see my Mom was exceptional.

Very rude people and always understaffed!!! Its not good to have 80 some residents and only 3 or 4 CNAs. Residents don't get the care they need like that. They DON and ADON are very rude to the residents and to their workers... I don't think that is right.

Aside from the ignorance and condescending attitude of some staff, most of them try to do the best they can with what pitiful and laughably few resources available. To task one or two of these uneducated and unhappy folks with such critical and complex duties is dangerous and unacceptable to the point of gross negligence. My treatment went well til my 5th day of detox...and I praise the doctors n staff. Except there was a really rude med nurse that singled me out n attacked me verbally n i had to check out as i didn't feel safe...in her care another nite.
